肺炎系列 (699) myocarditis/pericarditis following Comirnaty vaccination, involving adolescents aged 12 to 15 in HK

On 14 June 2021, the Food and Health Bureau, Hong Kong SAR Government lowered the age limit for receiving the Comirnaty vaccine to ≥12 years [1].

Between Jun 2021 and Sep 2021

A population cohort study in Hong Kong reported that, between 14 Jun 2021 and 4 Sep 2021, 33 Chinese adolescents who developed acute myocarditis/pericarditis following Comirnaty vaccination were identified. All cases are mild and required only conservative management. [2]

At the same time, between 13 Jun 2021 and 12 Sep 2021, the Department of Health of HKSAR received 29 reports of suspected myocarditis or pericarditis involving adolescents aged 12 to 15, after receiving the Comirnaty vaccine. [3-4]

Every week, the government of HKSAR had presented "Update on monitoring COVID-19 vaccination" in the press release [4]. For example, on 22 Sep 2021, In the preceding week till 11.59pm on September 19, the DH received six reports of suspected myocarditis or pericarditis involving adolescents aged 12 to 15. The six cases involved two males aged 13 and 15 respectively, two males aged 14 and two females aged 12 and 13 respectively. They experienced chest pains one to three days after receiving the Comirnaty vaccine. Their conditions are stable. [4] Then I try to gather those weekly information in 30+ weeks and organize in an open document.

A panel of health experts advising the Hong Kong government has recommended children aged 12-17 should get only one dose of BioNTech’s COVID-19 vaccine after reports of heart inflammation side effects. [5-7]



Between Jun 2021 and Jan 2022

Between 13 Jun 2021 and 30 Jan 2022, the Department of Health of HKSAR received 40 reports of suspected myocarditis or pericarditis involving adolescents aged 12 to 15, after receiving the Comirnaty vaccine. [3-4]

By 9 Feb 2022, there were 387,000+ adolescents aged 12 to 19 receiving the COVID-19 vaccine [8], that accounted for about 86.28% of that age group population.


Myocarditis and pericarditis

Myocarditis are diagnosed according to the Brighton Collaboration case definitions, which include:
clinical symptoms (cardiac or non-specific symptoms), elevated myocardial biomarkers (troponin T, troponin I or CK myocardial band), electrocardiographic, echocardiogram or cardiac magnetic resonance abnormalities and with other alternative aetiologies for symptoms excluded [9-10]
  • clinical symptoms
    • cardiac symptoms: Acute chest pain or pressure, Palpitations, Dyspnea after exercise, at rest, or lying down, Diaphoresis or Sudden death
    • or at least two non-specific symptoms: Fatigue, Abdominal pain, Dizziness or syncope, Edema, Cough
    • or symptoms in infants and young children: Irritability, Vomiting, Poor feeding, Tachypnea, Lethargy

By 9 Feb 2022, there were 325 adolescents aged 12 to 15 confirmed positive for COVID-19. And there were 550 confirmed cases of aged 12 to 17 [11].
