在相片中有提示：

• 有幾支膠針筒和膠手套 - 在醫院裡；
• 相的邊緣有條黑色長形物體 - 那是內視鏡，在內視鏡中心；
• 膠兜有紅色液體，在內視鏡中心有紅色液體，那是血；
• 膠兜有藍色液體，在醫療上和內視鏡中心，藍色液體是甲基藍 Methylene Blue；

從以上的線索，內視鏡、血、甲基藍，不需要勞煩福爾摩斯，谷哥 (Google) 在30秒內便找到答案，可能是染色內視鏡 (chromoendoscopy)，或是食道靜脈曲張出血 (variceal bleeding) 要用內視鏡止血。一大盤血，應該是後者，而我們可以用不同止血的方法：

1. 經內視鏡靜脈曲張結紮術(Endoscopic variceal ligation)；
2. 氣囊填塞 (Balloon tamponade) ，在1950年 Dr. Sengstaken 和 Dr. Blakemore 在外科醫學期刊發表了他們的氣囊填塞止血方法 [Ref 1]，所以現在一條膠管加氣囊便以他們的名字命名 Sengstaken-Blakemore tube [Ref 2]，其後有三種小變化：
• Sengstaken-Blakemore tube：有三個工作管，一個用於胃氣囊，一個用於食道氣囊，一個用於從胃抽東西。
• Minnesota modification of Sengstake tube：有四個工作管，第四個是在食道氣囊上面，抽口水和分泌物，以免吸入氣管；
• Linton tube：沒有食道氣囊，只有加大的胃氣囊。

一般氣囊都是打入空氣，用壓力幫助病人的食道靜脈曲張止血，有些地方會打液體入胃氣囊，包括水、亞甲藍 (methylene blue) 和x光造影劑 (x ray contrast)，用以確認位置和監測胃氣囊漏氣 [Ref 3]。

我親眼見過有幾個飲酒致肝硬化病人，在我面前嘔血，其中一位噴血噴到上天花板，用內視鏡止血，或是這條 Sengstaken-Blakemore tube，救了很多人。
